If you would like for us to send you a copy of your W2 or a paystub, the following information is REQUIRED:

• Full Name
• Date of birth
• Last 4 Digits of your social security number
• Employee ID with 24HR or your home club location
• Your address as it is known to 24HR
• Your current address if your address has changed

• W2 (include tax year)
• Paystub
